Chronic Disease Action Plan – 2022-2027

Project summary:
The Latrobe Health Assembly, in partnership with Latrobe Regional Health, Latrobe Community Health Service and Gippsland Primary Health Network have developed the Latrobe Chronic Disease Action Plan 2022-2027. The plan aims to act on a range of areas that will improve the experience of people living with chronic conditions, including early detection and identification, self-management and community-based treatment and care.
Designed in conjunction with local community and stakeholders, the Latrobe Chronic Disease Action Plan is a roadmap for the Latrobe Valley community.
Including both achievable and ambitious actions to drive improvements, the vision for the action plan is “To create an inclusive and integrated service network for all people with chronic disease. The voice of the individual or carer is heard, and they can access what they need at the right time to live a healthy and fulfilling life in Latrobe.”
The Plan has resulted in the development of several projects which Latrobe Health Assembly and partner stakeholders are working to deliver. Alongside this, Larter Consulting will lead an evaluation of the Plan in partnership with the Working Group and Steering Committee.
